A. A copy of the report when your child was diagnosed specifically with Autism Spectrum Disorder (ASD) (*Please note that a confirmation of
diagnosis (COD) from your child’s doctor will NOT be accepted by insurance).

B. A referral from your child’s doctor

4. Upon receiving these documents, insurance will approve an assessment to be completed by NO LIMITS ABA so that recommendations for services can be made
5. Once insurance reviews the recommendations they will provide an authorization for treatment to begin.
